On Going Problem

Thanks for the great advice!  I have been experiencing mild to moderate irriation at the uretheral opening for the past 4 months.  The first time it occured was exactly the morning after a night of sex.  We used condoms during 3 of the 4 times.  After a week I saw my PCP, he put me on nystatin cream (TID) because he was thinking it might be a yeast infection. Nystatin seems to help with the immediate pain but it comes back. The following week a had a slightly cloudy discharge and was put on 10 days of doxy. No help, so 10 more days of 500mg Leviquin.  I have had a negative swab for Gon/Chyl and a negative urine test as well!  No sores and no visible abnormalities have ever been seen by me, my PCP, a clinic doc and now my Urologist.  The irritation comes and goes depending on my level of activity. Most days it is irritated and slightly red w/ absolutely no swelling. My Uro gave me a digital exam, put me on Prosed and schedule me for a bladder scan in a few weeks. My partner assures me that she has no STDs.  Where do I go from here?

Should I still be using the Nystatin?

Are my docs over looking someting; bladder or prostate infection? Cancer? Anxiety?

Could it possibly be irritation from the condom that keeps getting re-injured with movement?

I awaite your advice. Thanks

Thanks for the thanks in advance, but I really can't help much.  No online provider can add much after the examinations, tests and treatments you have had.  Whatever is going on, the initial symptoms were not due to any STD; no infection can cause symptoms as soon as the next morning--it always takes 2+ days, and most cases of nongonococcal urethritis (NGU) take 7-14 days to cause symptoms.  Onset within hours suggests a physical irritaiton of some kind, e.g. from spermicide etc.

1) Yeast infection is unlikely as well.  But nystatin isn't a great choice for yeast anyway; many yeasts that cause genital infection in women (and less commonly in men) are resistant to it.  If your provider wants to try further treatment against yeast, I suggest fluconazole (Diflucan).  But yeast isn't a likely explanation and I don't think such treatment will make any difference.

2) I doubt anything is being overlooked.  Prostate inflammation might be possible, but that's a question for the urologist.

3) As I said above, physical irritation of some kind is much more likely than any infection.

Most important, with the negative tests plus the treatments you have had, you can be sure you have nothing that threatens your health or that of a future sex partner.  Most likely the symptoms will gradually fade with time.  But of course follow up with your own provider(s) if symptoms persist or you otherwise remain concerned.

Good luck--  HHH, MD
