Persistant Symptoms

I had unprotected sex with a girl(Pro) in Malaysia. The encounter did not last long and I stopped short of ejaculation. This occurred during the first week of December. During my flight back, I noticed that my legs were in severe pain. Probably due to an 18 hour flight and minimal movement. The pain persisted for a week. Then I started having yellowish discharge and a constant burning in my penis. I saw my dr. he told me that I probably had gonorrhea or clamydia. He prescribed me Levoquin  500 mg. for 10 days. During that time my discharge and burning went away. Since then I’ve had pains in my neck and what I can best describe has spasms or slight pain in my inner thighs. Every now and then I have less than a drop of clear discharge.  This has gone on for 3 months. I’ve been back to my doc many times. He put me back on Levoquin for 20 days and still no results. I’ve been tested for HIV, HSV-1 and 2. All negative. I’ve since been to a Urologist, who assures me that my STD is gone and that I may be just healing slowly.Ok, but 3 months is kind of extreme. Don’t you think ??  The only other symptoms that occurred in early December was, night sweats for about 2-3 days and did lose a couple of pounds. Since then I have gained the weight back and no more night sweats. I noticed that my neck, legs and clear discharge problems seem to re-occur a day after ejaculation. What is your opinion or theory on my symptoms? Am I wasting more time and money on seeing another Dr. ? I do want to get another HIV test soon, to rule that out. Since my first one was only 2 weeks after the incident.

Clearly you had urethritis, and the incubation period (more than a week) suggests nongonococcal urethritis (NGU), which could have been due to chlamydia or a nonchlamyidal cause.  Levaquin usually works well, but the risk of recurrent or relapsing symptoms is high (10-20% of cases) regardless of what drugs are used.  Your urologist undoubtedly is right; most such cases probably are due to inflammation, not persistent infection.  However, if the symptoms recur yet again, you should have a course of doxycycline or azithromycin instead of repeating Levaquin yet again.  Also, it might be a good idea to have a course of treatment against trichomonas, i.e., single dose treatment with metronidazole (Flagyl) or, preferably, tinidazole (Tindamax), which probably is more effective in men than metronidazole.

All of your other symptoms are entirely different.  They might be due to anxiety or other emotional reaction to the urethritis.  But they are not directly caused by urethritis and do not suggest any STD.

Bottom line:  If discharge returns yet again, get rechecked and talk to your provider about the additional treatments I suggested above.  You probably can pretty much forget the other symptoms, but of course talk to your provider about them if they continue or are bothersome.

Good luck--  HHH, MD

To: Scared to death of possible HIV
First, my Prays go out to anyone who find themself seeking medical advice.

I went on Vacation to the Dominican Republic Feb 13 - 19.  I did something very foolish.  I mixed sexual encounters with drinking alcohol.  I had protected anal sex with another male. I had unprotected oral sex as well where some semen got into my mouth.I had two HIV test done Feb 28,  less than two weeks later.  One was a blood test and the other an oral swab test.  They both came back negative.  However, I have had the following symptoms:

1a. The very first symptom was a wierd test in my mouth. It felt as though a some type of form had cover my tongue and checks.
1. Slight head pressure (not so much a headache) the past two weeks.  I feel like something is wrapped around my forehead.
2. I Could not sleep for about 6 days.  My Doc prescribed sleeping pills. No longer taking pills.
3. I get very tired for about 30 to 40 mintues during the day.  Then I get a burst of energy after resting.
4. I lost 7 to 10 lbs two weeks after sexual encounter.
5. Burning skin sensation after I eat in both my arms, hands and feet.  I have developed calious between my two baby toes and my second toe.
4. I have very dry scalp.
5. my mouth experience little sores which disappeared with in a day.
6. Aniexty attacks the second week.
7. Now my armpits sweat and I have this unusal musty smell.
8. I notice some light bumps that look like mosquito bites. One on my right arm and one on my check and two on my left arm.

I had a physical exam on March 15.  My Doc seems think everything is ok and that we have to wait until the 3 month period.  I am thinking about asking for a MRI/ pat scan for my head and requesting a referral for a dermatalogist. Do you think I am over reacting.  I am ready to sale everthing and give to charity. I will practice celebacy for a long time after this encounter. I know that God has all power in his hand.
