Background: The wife and I partially soft swapped w/ another couple (I gave unprotected oral to other girl, wife recieved unprotected oral from other guy).

About 3 weeks later, I start to have discomfort in penis shaft, burning/crushing feeling, sometimes the tip would have a cold feeling, and a feeling of a bit of urine still in UT after peeing.  Other than the pain, there is nothing that looks unusual on/around my penis.  My wife has no issues (other than a sorethroat/flu, but everyone we work in public schools where everyone seem to have a cold right now).

After I noticed some milky discharge (after 5 days of increasing discomfort) I went to doctor who precribed Levaquin.  I took 500mg for 10 days, dramtically reduced pain - put penis is still 'sensitive' (pain increases in shaft after it is handled). At end of 10 days of levaquin, doctor has switched me to doxcycline.  Im about half way thru that 200mg 10 day prescription as well.  Still I have tenderness and occasional feeling like urine is still in urinary tract, condition does not seem to be improving (but again, feels much better than before went on Levaquin).

I tested negative for Chlamydia and gonorria.  Is it it some other STD? Acute HIV infection?
I'm looking for any suggestions as to what this might be???

To your question:

It seems pretty clear you had nongonococcal urethritis (NGU), which is an STD.  I base that solely on the discharge, which clearly was abnormal; the significance of the other symptoms is less clear.  If I understand your description, your penis/urethra was not exposed to anybody's mouth or vagina (or anus) during your swapping event.  However, perhaps your wife was infected and you later acquired it from her, through either vaginal, anal, or oral sex.  A fair proportion of NGU is acquired by fellatio, probably because normal oral bacteria can cause the problem--that is, your partner(s) may in fact not have an STD in the usual sense.

At this point, having had both Levaquin and doxycycline, you can be sure there is no persisting infection.  Discomfort often continue after NGU is cured, perhaps due to an ongoing inflammatory reaction, but not active infection.  Other cases like yours probably are emotionally/psychologically based, although I don't assume that necessarily applies to you.  But most important, you can be confident you no longer have anything that will harm you, your wife, or any other sexual partner in the future.  (All this assumes, by the way, that the other members of your sexual 'team' have been informed and, to be safe, treated.)

No, there is nothing in this that hints at HIV.

So my wife could be infected w/o any symptoms at all?  Since I've gleaned that NGU can be transmitted thru saliva, how much contact should I avoid w/ my wife until she completes treatment? No kissing, no sharing food?

The partners of men with NGU normally are treated as a precaution.  In this instance, the odds are she has nothing that will harm her, or that will be transmitted back to you, either by oral or vaginal sex.  However, there are many unknowns and the practice makes sense, just to be safe.  A single dose of azithromycin (Zithromax) is all she needs.  Within 24 hours of taking it, you can safely resume any and all sexual practices.  Kissing is not an issue at all (before or after treatment) and I guess I don't understand why sharing food even comes up; it also is not an issue.

i have a bump on my under wear line. i dont kno what it is but it hurts real bad. i wrestle and i got felliculitous under my chin and it feels alot like that. it is right on my under wear line, its red and really sore all around it.i thought that it was just an ingrown hair becaus it started out like a pimple so i poped it then like a day later i thought i could see a hair under the skin so i put a warm rag on it for a little while then stuck a needle to it to try to get the hair out but then i poped the hed off of it and there was no hair. so i dont know if i shoul see a doc or if i should just give it a while and save the money.i would highly appriciate it if a doc would give me his oppinion

See a doctor as soon as possible--if you have fever, do it right now, even in the middle of the night. If no fever, see a provider first thing in the morning.
