Read the "inserts" of any medications he might be on, or that he's taken, to see what some of the bad side effects might be. This includes vaccinations that sometimes have "adverse" (bad) effects on some people.

Also check with holistic health practitioners (like a good acupuncturist, gentle chiropracter, etc.), books in health food stores, and other parents whose child has the same diagnosis (once you get one)-- about gentle treatments for his problems.
To me, it sounds POSSIBLY SOMEWHAT NEUROLOGICAL. Also, an "ABSCESS IN THE ANAL AREA" can sometimes cause this "anorectal incontinence". Did he ever used to "hold in" the stool, which can lead to not feeling the "urge to go" as strongly as he should.
Also, check into "Celiac Disease" as a possibility ("gluten" intolerance).
And a good child psychologist can be helpful if stress is a factor. But don't assume it's only stress, until (and if) you "rule out" all the other physical possibilities.
I like your idea of dietary changes for the better. Have you also tried acidophilus/bifidus ("pro-biotics"), in case he has some kind of intestinal infection? When you find out what he has, help others by telling us on this forum.
Do any of the other children in his school have a similar problem? Does he eat lunch at his school?

Again, good luck! Don't give up. Get a proper diagnosis by an EXPERT PEDIATRIC DOCTOR (Gastrenterologist, Neurologist, etc.)
